Passport Services
The Library is a passport acceptance facility, which means that members of the community can submit their applications for a passport at the Library.
Applications are available at the Library or can be downloaded at http://travel.state.gov/passport/forms.html.

We will be accepting applications every Monday from 2:00 pm to 6:00 pm, Tuesday from 2:00 pm to 6:00 pm, Saturday from 10:00 am to 3:00 pm, and Sunday from 12 noon to 3:00 pm. Appointments strongly recommended. Call 480-488-2286.

THERE IS NO PHOTO SERVICE AVAILABLE AT THIS TIME: PLEASE COME WITH PHOTOS TAKEN AT EITHER WALGREEN’S OR CVS OR AAA, ETC.

Applicants should come prepared with two passport photographs (or have photos taken at the Library), a current valid form of photo identification (state driver’s license, current U.S. passport, military ID card, etc.) and proof of U.S. citizenship (U.S. passport, certified birth certificate, certificate of naturalization, certificate of citizenship or consular report of birth abroad.) Payment may be by check or postal money order. Separate checks are required for the passport fee and the Execution fee. Credit cards are not accepted.
